Physiotherapy News : Odisha June 2014

Odisha: June 2014: With the State Cabinet now deciding to transfer Physiotherapy and Occupational Therapy to the Department of Health & Family Welfare, the practitioners in these fields are hopeful of a change, which has been evading them for last one decade.

Even as almost all the States and the Union Government have been handling Physiotherapy and Occupational Therapy under the Department of Health & Family Welfare, for the last decade these subjects continued to be under the administrative ambit of the Department of Women & Child Development (DoWCD) in Odisha.

The DoWCD has not given proper attention to these areas of concern, rued experts. With road accidents on the rise, the requirement of institutions for physiotherapy and rehabilitation has gone up, but Odisha is totally lacking in this front, said Kishore Chandra Mahalik, who has suffered seriously due to a road accident and getting rehabilitated in PGI, Chandigarh.

Physiotherapy and Occupational Therapy courses in pioneer institutes like the All India Institute of Physical Medicine and Rehabilitation, Mumbai, Post Graduate Institute (PGI), Chandigarh and Jawaharlal Nehru Institute of Post Graduate Medical Education and Research, Puducherry are dealt with by the Union Ministry of Health & Family Welfare.

Interestingly, the Odisha Clinical Establishment (Control and Regulation) Act, 1990 mandates registration of a therapy clinic under the Department of Health & Family Welfare (DoHFW). And the course curriculum can be better regulated by the Director Medical Education and Training, which is under the DoHFW, said a therapist.Now after a lot of dillydallying, the State Government has taken the decision in this regard, and experts in the field are hoping that facilities will grow as there will be focused attention.

While a number of NGOs and other institutions were waiting to get clearance for setting up physiotherapy and rehabilitation centres in the State, response from the DoWCD was lukewarm, said an active NGO member.

World Physiotherapy Day- Fit to Take Part - 2014

      “FIT TO TAKE PART “ –  Theme for World Physiotherapy Day 2014

   New materials launched for World PT Day 2014 – and the theme is 'Fit to take part'

WCPT has launched its materials to help physical therapists around the world mark this year’s World Physical Therapy Day on 8th September.
This year the theme is “Fit to take part”, emphasising physical therapists’ role in helping people with long-term illnesses or disabilities become independent and fulfil their potential.
Research indicates that people with a disability are less likely to be employed or to be playing an active part in society.  WCPT’s campaign materials this year emphasise the cost of this to individuals, their families, society and economies. Research indicates that lack of participation by people with disabilities costs some economies 7% of their gross domestic product.
But according to the United Nations “full participation by persons with disabilities will result in their enhanced sense of belonging and in significant advances in the human, social and economic development of society”.
WCPT’s World Physical Therapy Day materials this year demonstrate how physical therapists have a key role in promoting full participation by people with disabilities. The World Health Organization says that rehabilitation services such as physical therapy are a good investment because they “build human capacity”.
World Physical Therapy Day is marked every year by thousands of physical therapists around the world drawing attention to the profession's contribution in promoting global health. WCPT produces a toolkit of materials for WCPT member organisations and others to use in local and national campaigns. The toolkit includes posters, flyers, ideas, practical advice, resources on the contribution of physical therapists, stickers and t-shirt designs. 

“The wonderful thing about World Physical Therapy Day is that it can highlight the importance of every aspect of what we do and how we do it," said WCPT President Marilyn Moffat.
The materials produced by WCPT include an article by Marilyn Moffat which physical therapists and WCPT member organisations can send to publications in their country. It explains the role of physical therapists and the importance of participation by people with disabilities.
“The message has to go out to politicians and other policy makers that physical therapists are worth the investment,” she said.

Worldwide, there are at least 44 million people living with Alzheimer's and other dementias. It is found in at least one among 10 persons who are more than 80-year-old and one out of four persons who are above 90 years. In India, there are nearly 50 lakh dementia patients of which, roughly 70 to 80 per cent have Alzheimer’s. The Indian numbers are expected to double by 2030, and the costs involved are expected to increase three times.Everyone who has a brain is at risk to develop Alzheimer's—but everyone can help to fight it.  Now is the time to get involved. Alzheimer’s Disease (AD) is the most common type of dementia, in which the mental ability of persons gradually declines and reaches a stage where it becomes difficult for them to lead a normal life. With the disease progressing gradually, patients find themselves more and more dependant on their immediate family members for survival. The most striking feature of AD is the loss of short-term memory among patents. It starts as minor forgetfulness and progresses into a disorder in which persons can’t even identify their near and dear ones. Patients lose both visual and verbal memories such as forgetting telephonic conversations, names of recent acquaintances, misplacing objects and losing way in known surroundings.

Role of a Physical Therapist: 
Physical therapists (PTs) are health care professionals who diagnose and treat individuals of all ages, from newborns to the very oldest, who have medical problems or other health-related conditions that limit their abilities to move and perform functional activities in their daily lives.
PTs examine each individual and develop a plan using treatment techniques to promote the ability to move, reduce pain, restore function, and prevent disability. In addition, PTs work with individuals to prevent the loss of mobility before it occurs by developing fitness- and wellness-oriented programs for healthier and more active lifestyles.
Physical therapists provide care for people in a variety of settings, including hospitals, private practices, outpatient clinics, home health agencies, schools, sports and fitness facilities, work settings, and nursing homes. State licensure is required in each state in which a physical therapist practices.
The Physical Therapy Profession:
Physical therapy is a dynamic profession with an established theoretical and scientific base and widespread clinical applications in the restoration, maintenance, and promotion of optimal physical function. For more than 750,000 people every day in the United States, physical therapists:
Diagnose and manage movement dysfunction and enhance physical and functional abilities.
Restore, maintain, and promote not only optimal physical function but optimal wellness and fitness and optimal quality of life as it relates to movement and health.
Prevent the onset, symptoms, and progression of impairments, functional limitations, and disabilities that may result from diseases, disorders, conditions, or injuries.

The terms "physical therapy" and "physiotherapy," and the terms "physical therapist" and "physiotherapist," are synonymous.

As essential participants in the health care delivery system, physical therapists assume leadership roles in rehabilitation; in prevention, health maintenance, and programs that promote health, wellness, and fitness; and in professional and community organizations. Physical therapists also play important roles both in developing standards for physical therapist practice and in developing health care policy to ensure availability, accessibility, and optimal delivery of health care services. Physical therapy is covered by federal, state, and private insurance plans. Physical therapists' services have a positive impact on health-related quality of life.

As clinicians, physical therapists engage in an examination process that includes:
taking the patient/client history,conducting a systems review, and
performing tests and measures to identify potential and existing problems.
To establish diagnoses, prognoses, and plans of care, physical therapists perform evaluations, synthesizing the examination data and determining whether the problems to be addressed are within the scope of physical therapist practice.

Based on their judgments about diagnoses and prognoses and based on patient/client goals, physical therapists:
provide interventions (the interactions and procedures used in managing and instructing patients/clients),conduct re-examinations, modify interventions as necessary to achieve anticipated goals and expected outcomes, and develop and implement discharge plans.
Physical therapy can be provided only by qualified physical therapists (PTs) or by physical therapist assistants (PTAs) working under the supervision of a physical therapist.

Total Knee Replacement (TKR) surgeries are effective only when it is preceded and followed by physiotherapy:  If they don’t do physiotherapy and undergo TKR, then the outcome will be poor or they would struggle to recover Dr.M.Manjunath, Sports medicine specialist.

The last few years have seen an increase in the number of Total Knee Replacement (TKR) surgeries. Rough estimates suggest that TKR surgeries in India have grown at rate of 15 per cent every year.
 

In this backdrop, it is essential that prospective patients know the important role physiotherapy plays before and after TKR surgeries. Studies have shown that if patients undergo pre-physiotherapy sessions to strengthen their muscles around the knees, they definitely find it easy to recover after TKRs.

Patients with less muscle strength around the knees struggle hard to recover the full range of motion after undergoing TKR, physiotherapists say. Before surgery, physiotherapy should be prescribed to better prepare the knee and its surrounding muscles for surgery.
Muscle strength is often rated in the scale of zero to five, and patients who have muscle strength between zero and three should avoid TKR.

“Such patients should undergo pre-physiotherapy – first to strengthen their muscles and reach a muscle-strength scale of either four or five. If they don’t do that and undergo TKR, then the outcome will be poor or they would struggle to recover. Unfortunately, not many surgeons suggest pre-physiotherapy for such patients before TKRs,” laments sports medicine specialist M. Manjunath.

It is mandatory for the obese and those above 65 years of age to undergo muscle strengthening programmes before knee-replacement surgeries.
“For the elderly, at least four weeks of muscle strengthening is required to prepare them for TKRs. After they regain strength, their recovery post-surgery would be faster,” Dr. Manjunath explains.

Once the replacement surgery is over, post-physiotherapy sessions too are vital. The main aim of physiotherapy after surgeries is to make patients regain their full range of motion, attain muscle strength and proper gait. A minimum of 4-6 weeks of physiotherapy under a therapist, and later six months of exercises are needed to completely recover from the knee surgery.

Therapists point out that a lot of muscle is lost during knee replacement surgery and physiotherapy to regain strength is very important.

1. World Health Organization (WHO): "Physiotherapists assess, plan and implement rehabilitative programs that improve or restore  human motor functions, maximize movement ability, relieve pain syndromes, and treat or prevent physical challenges associated with injuries, diseases and other impairments. They apply a  broad range of physical therapies and techniques such as movement, ultrasound, heating, laser and other techniques. They may develop and implement programmes for screening and prevention of common physical ailments and disorders"

2. The International Labour Organization (ILO ): Physiotherapists and related associate professionals treat disorders of bones, muscles and parts of the circulatory or the nervous system by manipulative methods, and ultrasound, heating, laser or similar techniques, or apply physiotherapy and related therapies as part of the treatment for the physically disabled, mentally ill or unbalanced. The International Labour Organization (ILO) is the international organization responsible for drawing up and overseeing international labour standards. It is the only 'tripartite' United Nations agency that brings together representatives of governments, employers and workers to jointly shape policies and programmes promoting Decent Work for all.

3. International Standard Classification of Occupations (ISCO) is a tool for organizing jobs into a clearly defined set of groups according to the tasks and duties undertaken in the job.
The World Health Organization (WHO) has classified physiotherapists in professional group (ISCO Code 2264) and paramedical professionals have been classified in a separate entity (ISCO code 2240).

4. World Confederation of Physical Therapy (WCPT) : Physical therapists are qualified and professionally required to: -undertake a comprehensive examination/assessment of the patient/client or needs of a client group -evaluate the findings from the examination/assessment to make clinical judgments regarding patients/clients -formulate a diagnosis, prognosis and plan -provide consultation within their expertise and determine when patients/clients need to be referred to another healthcare professional -implement a physical therapist intervention/treatment programme -determine the outcomes of any interventions/treatments -make recommendations for self-management.

5. Delhi Council for Physiotherapy & Occupational Therapy Bill 1997 : “Physiotherapy” means physiotherapeutic system of medicine which includes examination, treatment, advice and instructions to any persons preparatory to or for the purpose of or in connection with movement dysfunction, bodily malfunction, physical disorder, disability, healing and pain from trauma and disease, physical and mental conditions using physical agents including exercise, mobilization, manipulation, mechanical and electrotherapy, activity and devices or diagnosis, treatment and prevention.

Maharashtra State OTPT Council: "Physiotherapy" means a branch of modern medical science which includes examination, assessment, interpretation, physical diagnosis, planning and execution of treatment and advice to any person for the purpose of preventing, correcting, alleviating and limiting dysfunction, acute and chronic bodily malfunction including life saving measures via chest physiotherapy in the intensive care units, curing physical disorders or disability, promoting physical fitness, facilitating healing and pain relief and treatment of physical and psychosomatic disorders through modulating physiological and physical response using physical agents, activities and devices including exercise, mobilization, manipulations, therapeutic ultrasound, electrical and thermal agents and electrotherapy for diagnosis , treatment and prevention.
